You will need the following Forms (All 3 original forms are needed to file)

  • Notice of Petition (signed by either an attorney or court clerk)
    After the above forms are completed the forms can be served 10-17 days prior to the court date. These forms can then be dropped off to the Lancaster Town Court office along with a $20.00 filing fee.
  • Affidavit of Service
